A 45 year-old female presented themselves to our office with a chief complaint:
“I need to complete my root canal.”
Clinical examination revealed the presence of a temporary restoration on tooth #21 with a previous access cavity. The tooth was normal to percussion. Probing and mobility were within normal limits.
Radiological examination revealed the presence of a separated file in a very narrow and calcified canal. There was also thickening of the PDL space.
The diagnosis was: Previously initiated therapy and asymptomatic apical periodontitis..
The treatment options that were offered to the patient were:
The patient consented for root canal treatment.
The procedure was completed in 2 visits with placement of Calcium Hydroxide as an intra-canal medicament.
During the first visit, the Terauchi File Retrieval Kit was employed in order to create a platform to get direct access to the broken instrument, which was vibrated out of the canal.
Biomechanical instrumentation of the canal followed using the XP-3D shaper and finisher files. Calcium Hydroxide was placed as an interim medicament and the tooth sealed with a temporary restoration.
Treatment was completed 10 days later. The canal was obturated using Bioceramic Sealer and Bioceramic Gutta Percha, offering superior biologic sealing and biocompatibility. The access cavity was sealed with resin composite.
